Observation

0165(3)(a) - Licensor observed two exposed bolts on the play kitchen in the preschool room due to missing pieces posing an injury hazard. 0165(3)(e) - Licensor observed a hole in the carpet and plywood in the corner of the stage by the window which poses a tripping hazard. 0165(3)(f) - Licensor observed cubbies/shelving to not be secured to the wall to prevent tip hazard. (note: applicant was working to anchor shelving/cubbies during inspection as time allowed) 0165(5)(a) - Licensor observed outlets in the "lunch room" to not be tamper resistant type. 0165(5)(b) - Licensor observed bathroom outlets by sinks to be tamper resistant but are not the GFCI type.
